palliative careEFM focuses on the individual. With each client we are inquisitive to know who this person really is, what makes their heart sing, and how we can assist this person to live the end of their life the way they want to.
 
Elder Family Matters can assist with the practical aspects of palliative care. These aspects may include:
  • Setting up a safe home to maximise independence
  • Supporting with tasks of daily living e.g. personal cares, shopping, meal preparation, laundry
  • Assistance to maintain connections with friends and family
  • Managing a dignified environment when friends visit to say their farewells
  • Providing companionship and a supportive ear with a person who cares
  • Assisting with medication management
  • Collaborative care planning alongside other health organisations, e.g. hospice, DHB funded care services
