This stage of care requires detailed care plans, liaison with other health care providers eg GPs, medical specialists, hospice. A team of Carers with a higher level of skill and experience are involved for Clients living with:

Dementia: Carers get to know the person, the links to their past, and what delights them.

Stroke: Care relating to the effects of a stroke may involve hoist transfers, feeding, elimination management.

Progressive Illness: A combination of skills, knowledge and emotional support for the Client and their family. The Client may have a progressive neurological illness such as Parkinson's Disease, or have Cancer.

Loss of Mobility: The use of wheelchairs and hoists within the care programme. Professional assessments determine the most appropriate equipment, and training is provided for Carers in the person's home to ensure safe use of equipment and consistent methods of handling.

End-stage Care: At this difficult time, EFM works closely with everyone involved - GP, hospice, family. The focus of the care is to ease the anguish for the Client and their family. We can be the constant presence in a home at this time, with medical and nursing professionals coming in and out. Family are freer to be with their loved one while personal cares, meals and other tasks are attended to by EFM.

Care Programme Examples:
    • Sessions: 3 or 4 sessions per day over 7 days
    • Overnights: When a Client needs assistance during the night
    • 24 hour care: A team of Carers covering day and night sessions
